Are you ready to make a meaningful impact in patient care? We are seeking a dynamic Pharmacy Extern - Medication Historian to join our dedicated team of healthcare professionals. This is your chance to work closely with skilled providers and pharmacists in a fast-paced, innovative environment focused on medication history collection and reconciliation.

What You Can Expect

  • Attention to Detail: Conduct independent work with precision in medication history collection.

  • Prioritize Patient Care: Identify and prioritize recently admitted patients for timely medication reconciliation.

  • Engage with Patients: Use strong interviewing skills to gather comprehensive medication information from patients or families.

  • Compile Medication Lists: Create detailed lists of home medications, including dosage, frequency, and last dose taken.

  • Provide Superior Service: Respond promptly to requests from physicians, staff, and patients, ensuring high-quality service.

  • Input Data Accurately: Enter medication history into the electronic health record with care.

  • Track Your Impact: Maintain records for intervention tracking and reporting outcomes.

  • Collaborate and Communicate: Participate in staff meetings to share insights and address team needs.

  • Adapt and Thrive: Embrace workflow changes in our growing health system.

  • Additional Duties: Support the team by taking on other responsibilities as assigned.

Qualifications

  • Enrollment in or recent graduation from an accredited college of pharmacy

  • Valid Idaho pharmacy intern/extern license

  • Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross
