St. Luke's Health System Care Advisor- BSN RN in Boise, Idaho

Apply today and #FindYourSelfHere St. Luke’s Health System in Boise is seeking an experienced nurse that is passionate about helping patients through telephonic patient advisory care.

What is so special about this position?

The Care Advisor uses assessment skills and appropriate risk assessment tools to identify patients with actual or potential health care needs that would require post-acute care coordination. This would include establishing mutual goals based on the patient's needs to evaluate, compare, and interpret ambiguous and changing clinical patient data. Working with the patient to educate and make sure they understand their health needs, discharge medications, upcoming appointments and educating them on infection prevention at home.

Strong team of physicians and multidisciplinary care team members to assess patient needs and develop a care plan. You will work with a team of RN's, Social Workers, Respiratory & Physical Therapists, and Health Coaches!

Schedule

Monday - Friday, 8am - 5pm. This position is extremely independent after training and has a team to partner in patient goals to help contribute to your success in this position. Must be able to manage and organize their own day to include quality audits and chart review. The position is Hybrid (50 on site /50 remote) Must be remote in the Boise Metro area. The in-person work site location is the Boise Plaza 4.

What you can expect:

  • Works with the patient, physician and care team, to create and implement a care plan that addresses the identified needs.

  • Will serve as the primary contact, advocate and resource for patients, families and physicians.

  • Assesses patient's knowledge of their clinical condition(s) and provide education and self-management support based on the patient's unique learning style.

  • Will measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.

  • Performs ongoing education to patient and patient's family and/or care giver on diagnosis, prognosis, treatment and medications.

  • Serves as advocate when problems and issues arise.

  • Perform other duties and responsibilities as assigned.

Qualifications:

  • Education: Bachelor's of Nursing Degree (BSN)

  • Experience: 2 years’ RN experience- RN Case management, Critical Care RN, or social work highly preferred.

  • Licenses/Certifications: Current RN License in the State of Practice
