The Day Habilitation Provider serves as a role model and advocate; ensures individuals’ safety by providing consistent supervision and observation; promotes a nurturing learning environment in the Day Habilitation setting and community; implements Individual Life and Staff Action Plans; and provides training of adult living skills in a dignified and respectful manner. Performs tasks on-site and in the community that involves training developmentally disabled individuals in various life skill areas, which in turn, will promote fuller personal development, enjoyment and expanded independence, integration, productivity, and individualization. •Implement all day habilitation service plans at agreed upon sites. This will include transporting the individuals.•Meet with individual’s family, Day Habilitation Supervisor, and Care Manager when necessary to assess individuals’ Day Habilitation Plan to make sure it is current and appropriate.•Complete agency vehicles logbook appropriately.•Ensure that agency vehicle is returned in a clean condition.
Minimum Salary: 15.50 Maximum Salary: 15.50 Salary Unit: Hourly
