The MIRACLES OF A SECOND CHANCE Registered Nurse (RN) or Licensed Practical Nurse (LPN) will be currently licensed to practice in the state of Arizona and have one (1) year experience in an acute care facility or residential setting.

Duties:

1. Ensure that residents are afforded medical attention on a regular basis and that all doctors visits are documented in the clinical record.
2. Ensure that all doctors orders are followed without exception.
3. Serve as a liaison between medical staff, resident and/ or guardian and the clinical case management team.
4. Facilitate with the clinical case management team all necessary referrals such as physical therapy, occupational therapy or any other treatment modality required by the resident.
5. Provide in-service training to MIRACLES OF A SECOND CHANCE staff concerning medication and other issues relating to resident care.
6. Provide counseling to residents, families of residents or guardians concerning medication issues.
7. Additional duties shall include implementing physicians orders, medication monitoring and dispensing, nursing care and implementing residents interdisciplinary treatment plan, attend CFTs, treatment planning meetings along with daily charting of vital signs and treatment progress.
8. All other duties as assigned by the MIRACLES OF A SECOND CHANCE director.
