Position Overview:
Under the supervision of the Clinical Manager, the Occupational Therapist is responsible for the delivery of appropriate, adequate, effective, efficient direct patient care services assigned by the Primary Care Clinician and approved by the Clinical Manager. Responsibilities include implementation of the physician plan of care, obtaining physician orders, preparing aide plan of care, supervision of aides, and coordination of care within an interdisciplinary team and participating in effective utilization management and control of patient supplies. Consistently provides care based on the age-related needs of the unique population with consideration of their development/psychosocial needs, environmental safety, educational needs, communication style, and level of understanding. The Occupational Therapist is accountable to the Primary Care Clinician for ensuring that the plan of care is implemented following professional standards of care and agency specific protocols. May provide direction and clinical supervision to an Occupational Therapy Assistant.

PRINCIPAL ACCOUNTABILITIES
In addition to the key accountabilities listed, employees working in this position are expected to accept and perform other duties as requested. All employees are responsible for integrating security practices in daily work by appropriately using and safeguarding SH IS resources in compliance with SH policies and procedures.
Occupational Therapy
• Implements general occupational therapy clinical procedures including, but not limited to: assessment and instruction in home safety and equipment needs, activities of daily living (ADLs), modification to promote self care, assistive devices, home exercise programs and dysphasia training per agency policy and procedure. Delivers direct patient care in accordance with all home care state/federal and accreditation standards including compliance to Medicare homebound and skilled care criteria where applicable
• Utilizes effective physical and psychosocial assessment skills, performs and documents multi-system assessments including OASIS
• Identifies and documents realistic, measurable and appropriate patient goals in collaboration with patient/caregiver
• Appropriately implements and updates the plan of care in a timely manner in accordance with accepted professional standards of practice in collaboration with the Primary Care Clinician and communicates communicates significant changes to MD in a timely manner
• Seeks appropriate consultation for insufficient progress towards patient identified goals
• Provide the patient with effective patient/caregiver instruction according to patient/caregiver learning needs
• Coordinates care with other clinical and clerical team members to meet patient plan of care and/or agency mission and objectives and documents coordination in the clinical record per policy
• Notifies Primary Care Clinician or Clinical Manager in a timely manner of emergency, unexpected or significant changes in the patient medical condition
• Demonstrates knowledge and appropriate utilization of internal and community resources to adequately meet patient needs
• Maintains positive working relationship with physicians in communicating clinically relevant information and progress towards discharge goals
• Consistently practices and teaches patient/caregivers infection control techniques in accordance with Infection Control standards
• Demonstrates appropriate and adequate clinical judgment in the implementation of patient care and is able to perform adequate problem solving in clinical situations
• Prepares the aide plan of care per policy
• Performs home health aide supervisory visits per agency policy and procedure
• Recommends alternative treatment modalities when clinically appropriate
Quality Assurance and Performance Improvement: (includes documentation and regulatory standards) including participation in peer audits as scheduled.
• Demonstrates and maintains OASIS and Integrated Care Management competency
• demonstrates knowledge of and actively participates in the Quality Assurance and Performance Improvement Program (QAPI) as assigned by the manager
• Performs peer audits as assigned
• Completes all documentation in a timely manner including consents, billing and clinical forms per agency policy and procedure
• Monitors and ensures positive patient outcomes for each plan of care intervention
• Demonstrates knowledge about and obtains updated information regarding all home care state/federal regulations and CHAP accreditation standards
• Demonstrates commitment to quality improvement through attendance to offered and/or mandatory staff in-services for improved operational and/or clinical functions
• Participates in the review of patient, physician and referral source customer satisfaction issues with the members of the clinical team on a regular basis and reports potential problems to the supervisor and facilitates prompt resolution
• Handles patient complaints and unusual occurrences diplomatically and promptly and complete required paperwork according to agency policy and procedure
• Maintains regulatory personnel requirements and submits all necessary documents in a timely manner including health requirements, updated CPR and professional license and completion of mandatory orientations and in-services
• Maintains job specific clinical competency levels as required by the agency
Fiscal Responsibility (includes productivity, HHRG rating, supply management, utilization review)
• Implements visit schedules based upon patient need and payor authorizations
• Accepts clinical assignments as designated by the manager
• Promotes judicious use and cost effective practices for patient supplies utilization
• Meets designated visit productivity standards for patient visits and apprises manager in a timely manner of variances in caseload and issues impacting productivity
• Submits all documentation in a timely manner and in compliance with relevant agency policies and procedures
• Identifies and recommends methods to reduce costs without compromising patient care outcomes
SKILLS AND KNOWLEDGE
Exhibits excellent oral and written communication skills: with legible handwriting for documentation of patient records in a clear and concise manner.
Basic understanding of home care services, clinical standards of care and practice, Title XXII, Medicare Conditions of Participation, CHAP home health accreditation requirements, QI outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payors; good working understanding of budget and other financial reports.
Self-starter with a high degree of initiative, motivation, flexibility, energy and creativity.
Ability to form harmonious working relationships with internal and external customers.
Demonstrated leadership and negotiation skills and the ability to manage resources for patient care in a cost effective manner.

Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.
Is responsible for maintaining all required licensure and certifications.
At orientation and annually thereafter, must be able to meet Sutter Care at Home core competencies for position and area of specialty, as appropriate.
Must have reliable transportation.
EXPERIENCE
One year of professional experience required. Required
One year of experience in a home health and/or hospice position preferred. Preferred
Direct patient care experience within the past five years required. Required
EDUCATION
Education Level: Master's Preferred
Additional Requirements/Area of Study: Mastes in Occupational Therapy

Pay Range is $53.49 to $71.99 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
