Qualifications

OBJECTIVE/PURPOSE:\ The Coordinator is responsible forestablishing communityconnections to enhance activities and opportunities for DS clients.The Coordinator organizes, develops, implements, coordinates, supervises, and monitors the provision of services to DS clients in the community. The Coordinator will train and supervise staff that work with these individuals andwill promote individual choice and meaningful services for each client.

MAJOR RESPONSIBILITIES:

PROGRAM DEVELOPMENT:Develop a weekly schedule for DSPs and clients. Adjusts schedule as necessary for vacations, call outs, and additional DSP and client needs.In collaboration with the Manager, develop community activities and resourcesfor program participants.The Coordinator will seek out new activities regularly enhancing client opportunities in the community.Activities should include leisure, educational, volunteering, health and wellness, social interactions, and more. Monthly client trips will be explored.\ Ensure that all waiver services are scheduled and being received by everyone with community support hours, and any waiver changes are reported promptly to the Division Director.Review Monthly Summaries and data collection systems completed by the staff and make certain that services are provided as outlined in the individuals Individual Service Agreement (ISA). Attends all Developmental Services and UCS meetings necessary to promote coordination and communication.Serves as facilitator for client ISA meetings with regards to community support goals and supports. Writes person centered client goals for ISAs.Facilitates client meetings for problem solving, changes in schedules, and other support necessities. Evaluate consumer goals for effectiveness and recommend changes to Case Manager/Qualified Developmental Disability Professional (QDDP) when needed. Train staff in methods and strategies needed to implement ISA outcomes and supports. Ensur that all regulations are adhered to.Develops individualized community connections for each individual which are reflective of the individuals ISA plan and ensures implementation.Ensures all staff are following regulations and addresses staff issues when they arise. Develops clear and meaningful expectations for all staff.Creates systems whereby all staff are appropriately trained on client support plans.Complete administrative duties as assigned.

Provide direct support when there are staff shortages. The Coordinator will be familiar with all community activities and be able to fill in when necessary. Participates in the DS on-call rotation as required.\ SUPERVISION AND DEVELOPMENT OF PERSONNEL:Responsible for overseeing the Community Support Program.
