Position Description

TITLE Clinician

REPORTS TO Clinical Services Director

POSITION CLASSIFICATION Regular Full-Time, Non-Exempt

SALARY                                               $37.14 –  $52.28

POSITION SUMMARY

Responsible for providing quality, comprehensive health care within the Health Connection clinics.  Incumbent will work closely with management to ensure medical standards are met in compliance with funding source guidelines, company policies and procedures. Incumbent will coordinate, plan, and manage staff, overseeing day-to-day operations of specific site insuring proper accountability consistent with performance standards and company policies .

 

skills & education requirements

Current Nurse Practitioner, Nurse Midwife, or Physician Assistant license and certification in the State of Indiana.  Experience in Public Health and/or Women’s Health preferred. Individual must be able to satisfactorily perform duties and;

  • Effectively communicate both orally and in writing; have the ability to write reports and business correspondence; have the ability to e ffectively present information;

  • Cooperate successfully as a member of a team;

  • Possess good organizational skills, with sensitivity to details; and

  • Have knowledge of computer operations and possess skills preferably with MS Office.

  • Must possess a valid driver’s license and have access to a personal vehicle with required liability insurance for use in business related travel.

  • Must ensure efficient operations and present a professional image in conduct, attitude and attire.

  • Must be philosophically compatible with the mission of PACE.

    Essential Duties & responsibilities

     

  • Develop and maintain a basic understanding of the corporation to include its programs, services and policies.

  • Assess and refer patients regarding other medical or social problems to appropriate sources of care.

  • P articipate in the maintenance of quality care standards in the clinician role.

  • Perform health screenings and evaluate test results.

  • Implement medical protocol and participate in the maintenance of medical standards in the clinician role.

  • Maintain and review appropriate records, including documentation of contact, referrals, and follow-ups.

  • Provide guidance to ensure the maintenance/repair of facilities, and ensure a work-safe environment

  • Supervise assigned staff, implementing Professional Development Plans and provide on-going training.

  • Ensure proper care and use of supplies and equipment.

  • Conduct individualized education and counseling; and assist with recruitment and community education.

  • Ensure patient confidentiality and develop procedures to protect records at all times.

  • Travel to other sites within our service area.

  • Attend all assigned training and meetings.

  • Submit timely, accurate and complete reports.

  • Comply with company, state, and federal regulations.

  • Provide on-going communication and information sharing with staff, participants, and community members.

    Physical/Mental Demands

    Requires sitting, standing, stooping, bending and reaching with manual dexterity sufficient to operate standard office machines such as computers, fax machines, calculators, copy machines, telephone and other office equipment.  May require lifting up to 40 pounds.  Requires normal range of hearing and vision.

    Must be able to handle diverse work problems on a daily basis.  Requires ability to set priorities and work schedule yet adjust to changes necessitated by last minute assignments and deadline requests that may prove stressful.  Consistent pleasant attitude is necessary with personal maturity as an important attribute.  Must relate and interact with people at all levels of the company and in a culturally diverse environment.
