Job Description:

JOB SUMMARY

The Activities Coordinator Senior Services will plan activities for all residents based on the comprehensive assessment and care plan and the preferences of each resident. There will be an ongoing program that supports residents in their choice of activities, both facility-sponsored group and individual activities and independent activities, designed to meet the interests of and support the physical, mental, and psychosocial well-being of each resident, encouraging both independence and interaction in the community.

ESSENTIAL JOB FUNCTIONS

  1. Plans activities that are appropriate to the needs of the residents (includes but are not limited to: small and large group programs, individual pursuits, sensory activities, clubs, special interests, spiritual and religious activities, one to one activity, community outings)

  2. Prepares a monthly calendar of activities that is posted in prominent areas and provided to every resident.

  3. Prepares a monthly staffing schedule that includes evening and weekend and holiday coverage.

  4. Ensures that all documentation requirements are met. (MDS, assessments, care plans)

  5. Develops a system to record resident participation/attendance.

  6. Orders appropriate supplies and equipment for the department

  7. Plans a monthly resident council meeting (and any other resident committees such as food committee) and is the staff liaison for the resident council president and other resident executive committee members. Takes meeting minutes and ensures that all resident concerns are addressed.

  8. Arranges regular entertainment, parties, seasonal programs, special events including community outings.

  9. Ensures that the facility is appropriately decorated as per the holiday/season.

  10. Coordinates community grassroots programming and serves as facility representative at local events.

  11. Adheres to all facilities and corporate policies.

  12. Coordinates the development of the in-house newsletter with input from residents, employees, and guests; oversees the distribution of the newsletter to residents, families, and community groups.

  13. Regular attendance is required in order to carry out the essential functions of the position.

  14. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.

ADDITIONAL DUTIES 1. Other duties as assigned.

JOB QUALIFICATIONS

EDUCATION

For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

Minimum Required: Possesses a high school diploma, GED, or equivalent combination of education and experience and holds Activities Director Certification in the State of Wisconsin or Has a bachelor’s degree in Occupational Therapy and is licensed as an Occupational Therapist in applicable state or Has an associate degree in Occupational Therapy and is licensed as an Occupational Therapist Assistant in applicable state

Preferred/Optional:

Bachelor’s degree in therapeutic recreation

EXPERIENCE

Minimum Required: Exposure to the aging process, dementia, and chronic diseases of the elderly and evidence of completion of academic/clinical in the geriatric field.

Preferred/Optional: Working knowledge of personal computer and software applications used in job functions.

CERTIFICATIONS/LICENSES

The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.

Minimum Required: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire. Certified Nursing Assistant Course completion within 6 months of hire.
