Hillside Family of Agencies Community Based Treatment Provider II in Batavia, New York

The Community Based Treatment Provider II provides in home and community-based therapeutic interventions to youth and families to assist in behavioral and mental health symptom management. This position provides Community Psychiatric Supports and Treatment (CPST) within Child and Family Treatment and Support Services. May provide Psychosocial Rehabilitation (PSR) and Planned Respite (PR) within Child and Family Treatment and Support Services as well as Home and Community-Based Services programs, depending upon the region.

This position is full-time, 40 hours per week. After the initial training period, this is a primarily mobile role and you can set your hours according to your caseload! Flexibility for some evenings and weekends to support client needs/availability is required, but on-call is not required for this position.

Essential Job functions

  • Assess youth and family needs and create individual service plans to meet those unique needs.

  • Update the individual service plan regularly in collaboration with the child, youth, and family and multi-disciplinary treatment team.

  • Complete all required documentation in the Electronic Medical Record, including but not limited to, appropriate screening tools, treatment plans, safety plans and progress notes.

  • Provide trauma-informed interventions to increase functioning in the home and community to achieve service plan goals. Areas of focus may include, but are not limited to: psychoeducation, mental health symptom management, emotional regulation, suicide prevention, healthy relationships, family routine and structure, harm reduction, task completion, communication, socialization, interpersonal behavior, community activities, activities of daily living, problem-solving, managing money, and positive coping skills.

  • Collaborate with other informal supports, community, and mental health/behavioral health providers to support service plan goals and objectives.

  • Manage crises and de-escalate challenging situations.

  • Assess and facilitate referrals to necessary services and community resources in conjunction with the treatment team, as needed.

  • While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.

Education & Experience

  • Bachelor's degree in human services related field required.

  • Minimum 2 years of experience working in children’s mental health, addiction, education, foster care, child welfare, juvenile justice or other related human services field.

SPECIAL REQUIREMENTS

  • Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

  • CPR and First Aid certification required or obtained within 60 days of hire.

Physical Demands & Work Environment

  • Must be able to work a variable scheduling including evenings and weekends

  • The following daily physical demands are required:

  • Sitting (up to 6 hours)

  • Occasional standing (up to 2 hour)

  • Occasional walking (up to 2.5 hours)

  • Driving (several times a week up to 6 hours)

  • Continuous balancing (up to 8 hours)

  • Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)

  • Weekly lifting up to 10-15 lbs. on a daily basis

  • Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)

  • Occasional exposure to dust, fumes, gases, chemicals, or smoke is apparent

  • Ability to change positions as needed

SPECIAL CONSIDERATIONS

While Hillside is a restraint free environment, there may be times in a “life or limb” situation, that staff may be required to physically restrain clients weighing between 50 lbs. and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.

$17.50 Minimum Rate per hour, $25.49 Maximum Rate per hour, based on experience.
