Summary:

The Behavioral Health Home Nurse Care Manager (NCM) works as part of a multi-disciplinary team to provide holistic behavioral health coordination to patients. This includes providing education to teammates and patients regarding their medical and physical health needs as well as behavioral health needs. The NCM works to track department wide health goals including reducing the number of unnecessary emergency department visits, attendance of well checks with primary care providers and completion of required lab work. The NCM is an expert on navigating patient’s electronic health record and sharing necessary information with patients and team members alike. NCM works to coordinate and facilitate monthly medical consultation with the BHH medical director. At times, NCM joins patient appointments related to their health goals, coordinates with providers and provides educational and emotional support to patients as needed.

Responsibilities:

  • Medical Consultation-prepare and facilitate medical consultation with our medical director and teammates. This may include a case study, journal article review or general education topic.

  • Health Infonet (HIN) -Monitor HIN for notifications regarding BHH patients attending the emergency department. Notify team members and track BHH follow up with patients regarding visit. Provide education as appropriate regarding patient needs around emergency department visit.

  • BHH Quality Measures-Track well checks of BHH patients as well as A1-c lab work and provide education regarding these measures and their importance in overall health and wellness. Provide overall education and support to staff and patients regarding health and wellness.

  • Orient new hires to BHH NCM role-Meet with all new team members of BHH and share information about how to utilize the Nurse Care Manager as well as share information about our health goals, provide education about HIN and medical consultation.

  • Patient support, advocacy, collaboration, and appointments-Attend appointments with clients and/or providers as necessary. Provide education regarding patient diagnoses to patients and teammates. Coordinate with providers regarding client needs.

  • Provide education to team and BHH patients based on needs, initiating own education on needed topics to share. Prepare and present information to team at staff meetings.

  • Other Duties as assigned.
