Summary:

  • The Director, Penn Partners in Care (PPiC) skillfully and strategically leads care management services, including our Penn Integrated Care (PIC) program supporting 50 plus primary care practices from Penn Primary Care and CPUP's Departments of General Internal Medicine and Family Medicine. These services include transitional and longitudinal care management, support of patient's social needs via assisting access to community resources, and care goal achievement through patient activation and engagement strategies; thereby, actively managing a percentage of our managed lives population. Additionally, the Director is responsible for our mental health co-management program (PIC) in partnership with the Department of Psychiatry. The Director is responsible for the operations, performance improvement and outcomes of program services as well as attracting and developing a team of highly capable and committed people to ensure long-term sustainability.

Responsibilities:

  • People Management: Lead and develop the PPiC organization which includes a management team of Lead Care Managers and Administrative Leads and their teams: total of over 70 FTEs of RN Care Managers, Social Workers, Care Partners, Mental Health Providers and Intake Coordinators distributed across the greater Philadelphia region at 35+ practice locations Develop and oversee a practice based clinical pharmacy model in conjunction with health system pharmacy leadership that includes 4 pharmacy FTEs Responsible for overseeing the daily operations of the team and coordination of departmental and staff needs. Actively leads the recruitment process, orientation, training, development, and mentoring of new care management team members, partnering with PCSL Disease Teams and key stakeholder groups to develop disease specific education, new workflows and education modules. Responsible for leadership development, succession planning and performance of the care management team. Actively manages employee relations issues in collaboration with Human Resources as needed. Completes department performance review processes, soliciting input from others (practice providers and staff)

  • Clinical Oversight: Provides clinical oversight and guidance to the care management team. Assesses clinical competency and builds programs to maintain or augment deficits. Supervises all clinical activities of the care management team members

  • patient care, establishing standards of care, ensuring compliance with any payer and/or regulatory requirements (including Joint Commission requirements in mapped practice locations), and ensuring timely, complete, and thorough documentation. Oversees, coordinates, and monitors all activities to facilitate integrated & proactive care coordination for patients Collaborates with entity clinical leadership and PPC medical directors on new care models and refinement of existing program services Partners with health system and academic research and data science stakeholders to develop a framework for program evaluation and optimization Represents and advocates for PPC clinical and operation goals on health system committees for post-acute integration and social determinants of health

  • Assists PCSL, entity Operations and Quality, and practice transformation teams to assure care management requirements and timelines for Centers for Medicare & Medicaid Services (CMS) and other payer initiatives are met.

  • Collaborates with internal and external stakeholders to ensure regulatory and payer requirements and timelines are met.

  • Formulates and develops annual PPC operating budgets, as well as setting quality and productivity standards

  • Maintains standardized care management procedures, data quality monitoring plans, and prepares and develops reports with PCSL informatics team as needed.

Credentials:

  • Registered Nurse - PA (Required)

Educations/Experience:

  • Master of Arts or Science (Required)

  • MSN, MHA, MBA or related field10+ years of total relevant experience including:Over five years of supervisory or management experience, encompassing extensive employee relations expertise, within a nursing or similar healthcare environment.3+ years Experience in care/case management within a multidisciplinary team for patients with chronic diseases, in settings such as hospitals, ambulatory care, or insurance companies preferred.
