Job/Position DescriptionProgram Specialist VIUnder the general direction of the Manager for the Diabetes and Cardiovascular Disease (CVD) Branch in the Chronic Disease Unit, serves as the team lead and program coordinator of the Diabetes Prevention and Control Program (DPCP). Performs highly advanced (senior-level) program planning, development, implementation, and management of DPCP activities. Provides technical assistance to program staff, internal and external partners, and leadership on all topics related to diabetes. Assigns work to other team members. Supports statewide implementation of cross-cutting approaches to address diabetes prevention and management using evidence-based strategies. Assists in grant application, implementation, management, reporting and evaluation activities. Develops contract scopes of work, monitors contractor activities and budgets, ensures contract activities are conducted and reviews for quality, and provides technical assistance to contractors and other DPCP staff that work with contractors. Develops and reviews educational materials on evidence-based best practices and approaches to prevent or manage diabetes for a variety of audiences. Consults with stakeholders, oversees contracts, and implements directives and projects designed to promote delivery of evidence-based services and education on a statewide level for people diagnosed with prediabetes or diabetes. Assists management and section leadership with completing legislative, division and section level assignments. Works under minimal supervision, with considerable latitude for the use of initiative and independent judgment.Essential Job FunctionsServes as Team Lead for the Diabetes Prevention and Control Program (DPCP). Oversees advanced programmatic work to support the planning, development, implementation, and strategic direction of DPCP, including coordination of daily and long-term activities of program staff, budget accountability, reporting requirements and evaluation of programmatic activities and impacts. Establishes program goals and objectives, and develops activities, schedules, priorities, and standards for achieving them. Implements and manages advanced special project initiatives funded through general appropriations, cooperative agreements, or other funding sources. Serves as a back up for the manager, represents manager in meetings, conferences etc, and may need to fulfill other related duties. (25%)Leads the team in developing grant applications, activities, budgets, work plans and progress reports. Identifies priorities for the federal grant and works with DPCP staff and contractors to achieve the aims of the federal grant and of the DPCP. Identifies potential contractors and develops contracts, including writing scopes of work, budgets and deliverables. Monitors contracts, provides technical assistance to contractors and staff as needed, and ensures appropriate and complete spend down of contract funds. (40%)Works with the Diabetes and Cardiovascular Disease Branch Manager in preparing reports, bill analyses, cost estimates, and responses related to legislative actions. Creates reports, briefings, fact sheets, summaries, and other information sources as requested. Prepares and presents information at meetings, conferences and trainings. Gathers and interprets information to identify best practices for the program and makes recommendations to the Diabetes and Cardiovascular Disease Branch Manager.
