Job Overview
The Clinical Social Worker I is a licensed clinician that conducts psycho-social assessments and interventions with patients/families and serves as a liaison with multi-disciplinary care team and external community agencies to implement interventions. This position will cover/focus on Young Mom's Clinic

Qualifications
Education: Master's degree in Social Work Experience: Minimum of 1 year of post-Master's full-time work experience providing psycho-social assessments and interventions to patients and families. Licensure(s):Licensed Social Worker (LSW) in the state of Colorado (CO) This position may require a BLS certification from the American Heart Association. Develops, implements, and monitors service/treatment interventions that provide a variety of services to meet patient/family needs. May serve on department and hospital-wide committees as directed. Develops a therapeutic relationship with the patients and families to assist with the social, financial, and psychological problems associated with hospitalization and/or outpatient treatment. Develops and maintains relationships with peers within and outside of the department as well as community resources. Conducts initial needs assessment to gather information on the patient/family's medical, social, and financial history. Serves as a liaison with multi-disciplinary care teams and external community agencies to identify, implement, and monitor treatment goals.
