Overview

We are looking for a highly organized and motivated Palliative Project Coordinator to join our growing Value Based Healthcare (VBHC) team. This position will directly report to the Director of New Program Implementation and is responsible for providing excellent project coordination support and contributing to the overall improvement of quality and cost effectiveness of care. You will work closely with cross functional teams, including clinicians, data analysts to achieve project goals on time and within budget.

  • Assist the full project lifecycle for assigned VBHC initiatives, including planning, execution, monitoring, and evaluation

  • Effectively communicate project information and expectations to diverse audiences

  • Build and maintain strong relationships with internal and external stakeholders

  • Maintain accurate and complete project documentation

  • Identify opportunities for process improvement and innovation within VBHC projects

About You

  • Bachelor's degree in project management, healthcare administration, or a related field

  • Prefer prior experience in project management, preferably in a healthcare setting, but not required

  • Understanding of value-based care principles and methodologies preferred but not required

  • Excellent communication, interpersonal and collaboration skills

  • Ability to manage multiple priorities and deadlines effectively

  • Proficient in MS Office Suite (Outlook, Excel, PowerPoint, Word)

  • Familiarity of project management softwares (e.g., Monday.com)

  • Experience with data analysis and reporting tools preferred, but not required

  • Ability to travel 25% of the time

We Offer

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

  • Salary Range: $45,000-$55,000
