8:30am - 5pm/ Full Time/ 40 Hours
We are seeking a capable Transplant Coordinator / RN who has acquired broad experience in caring for patients, and has developed a sound understanding about the care of a particular patient population
The Transplant Coordinator enhances internal and external quality of services through quality assurance and improvement practices. T
Coordinates the collection of clinical data and patient samples for research studies. Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning.
Participates in developing the interdisciplinary plan of care ensuring essential protocol medications, pertinent diagnostic tests, and frequency are ordered, completed and reviewed by physician.
Reports signs/symptoms of possible rejection immediately to physician and care team. Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care.
Coordinates communications with the outpatient transplant population such as transplant clinic visits, community referrals, telephone consultations, arranging for long-range beepers, outpatient treatments, and subsequent hospital admissions.
Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community.
Participates in daily patient rounds/huddles with transplant team. Participates in collaborative, interdisciplinary research activities related to transplant patients. Performs related responsibilities as required.
MINIMUM QUALIFICATIONS
An Associate's degree in nursing. 3 years of recent, clinical, medical-surgical nursing experience.
All nurses employed in roles requiring an unencumbered nursing license must have a valid, active license or temporary permit approved by the Georgia Licensing Board.
BLS certification required.
