Position Summary

The Care Coordinator’s primary responsibilities are to facilitate care management and coordination of clinical activities for chronically ill patients. The Care Coordinator also coordinates healthcare services across the patient care continuum during acute, chronic and long-term stages of illness for defined patient populations. This includes utilization management, transitional care planning, psychosocial and functional status assessment, patient advocacy, education for the patient/family, and monitoring quality indicators to demonstrate outcomes resulting from the service provided. Utilizing technological tools, Care Coordinators engage in patient outreach to ensure timely and efficient care delivery across the continuum of care.

The Care Coordinator will also work cooperatively with primary care teams to serve the needs of patients by assisting patients to safely and effectively navigate through the care continuum in order to assure comprehensive assessment of the patient’s plan of care, attainment of desired goals and outcomes, and to promote continuance of care. The Care Coordinator will use assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination.

Minimum Qualifications

Education:

  • High school diploma required.

Minimum experience required:

  • A minimum of three (3) years of experience in clinical practice in an ambulatory setting required; or equivalent combination of education and experience in a hospital inpatient, outpatient, behavioral health, or social service environment.

Qualifications:

  • Knowledge of ambulatory clinic protocols and NCQA , OSHA , JACHO , PCMH , and Care Coordination requirements.

  • Ability to train and educate patients regarding Treatment Plans and Patient Safety processes.

  • Functional knowledge and understanding of the mechanisms of Electronic Medical Records ( EHR ).

  • Certified or licensed clinical professional; CCM (Certified Care Manager), CSS (Community Support Specialist), CMA , LPN , RN, NP, LPC , LCSW , or other approved clinical licensed professional preferred. Applicable certification or licensure must be current and active for the state of Georgia.

It is MSM’s Immunization policy that all Prospective Employees are required to provide proof that they are vaccinated against COVID -19 prior to commencement of employment.
