Description

Position Summary:The Program Coordinator has the decision-making responsibility for the overall day-to-day operation of the Day Habilitation Program. He/she ensures that the program operates in a manner in which is consistent with both the agency and program specific mission statements. The Program Coordinator ensures compliance with all regulations, agency policy and procedures, and operates within budgetary limits.

Essential Accountabilities:

  • Act as the leader in the program in all areas related to the operation of Day Habilitation. This includes supervision of all staff. Provide all staff with the support needed to maximize their performance.

  • Ensure that all program activities enhance an individuals independence, dignity and social responsibility, and personal valued outcomes.

  • Ensure that program is in compliance with all agency policy and procedures, as well as all Federal, State and local standards and regulations.

  • Develop and ensure implementation of all operation systems including evacuation procedures.

  • Ensure that the weekly census is completed in an efficient and timely manner.

  • Develop and ensure implementation of: program plan, goals and objectives, which are consistent with agency philosophy and mission, and focused on the development of each participants maximum potential.

  • Ensure implementation of appropriate systems for: screening, admissions/discharge, referrals, ISP review, organization and staffing patterns.

  • Participates in Day Services Admissions/Discharge Meetings regularly.

  • Conduct staff meetings on a regular basis.

  • Participate as a member of committees as appropriate and as assigned.

  • Maintain ongoing curriculum development and ensure implementation.

  • Responsible for developing working relationships with other providers, with the object of ensuring that opportunities are made available in the community for movement of individuals to less restrictive settings.

  • Ensure the program meets internal standards of quality.

  • Develop, implement and monitor cost control measures within the Day Habilitation Program.

  • Monitor environmental plant issues and initiate corrective actions when indicated.

  • Develop a system that will ensure all staff receives required trainings (e.g., SCIP, AMAP, CPR, first aid, etc.)

  • Monitor participants progress by observing consumers/staff performing tasks and act as a role model for all staff during all program activities.

Other Responsibilities:

  • Must be able to physically intervene with participants when an individual may cause physical injury to self or others; assist participant in responding to potentially dangerous situations (i.e., hand on hot stove, running into traffic); able to administer emergency first aid to participant.

  • Must perform all aspects of ADL for individuals who cannot perform such tasks independently (toileting, showering, shaving).

  • Must be able to physically interact with the program individuals, negotiate the environment independently including assisting in all SCIP interventions, emergency CPR and or first aid techniques and fire drills.

  • Maintain confidentiality at all times.

  • Other duties as requested.

Qualifications

Masters degree in Special Education, Rehabilitation Counseling, Social Work or related field preferred, Bachelors Degree required and at least two years supervisory experience preferred. Valid NYS drivers license required.
