Job Title: Case Manager - Critical NeedJob Description

Review admission reports and coordinate with hospital social workers and discharge-planning staff to ensure early identification of high-risk patients and timely discharge planning. Identify, facilitate, and advocate for appropriate organizational and community resources to meet the plan of care in a cost-effective, efficient, and timely manner. Analyze patient and program outcomes to identify improvements in program quality and cost-effectiveness of case management activities. The clinician must be self-motivated and able to learn in a fast-paced environment and work independently once orientation is completed. Coordinate and facilitate appointments, referrals, and follow-up care for patients within the ambulatory setting. Collaborate with healthcare providers to ensure seamless transitions of care and continuity of services.

Responsibilities

• Review admission reports and coordinate with hospital social workers and discharge-planning staff.

• Identify high-risk patients and ensure timely discharge planning.

• Facilitate and advocate for organizational and community resources.

• Implement plans of care efficiently and cost-effectively.

• Analyze patient and program outcomes for quality and cost-effectiveness improvements.

• Coordinate and facilitate patient appointments, referrals, and follow-up care.

• Collaborate with healthcare providers for seamless transitions of care.

Essential Skills

• 5 years of case management experience.

• Experience in inpatient case management and discharge planning.

• Ability to work independently after orientation.

• Proficiency in handling critical care and specialty RN tasks.

Additional Skills & Qualifications

• BLS certification.

• State License in VA.

• Experience with prior authorization and healthcare processes.

Work Environment

The position requires working onsite at VHC through Our Client's program. The working hours are from 08:30 AM to 05:00 PM with a commitment to work every other weekend and during holidays. The role involves working with two EMR systems: KP EMR and VENDOR EMR. The role offers great health benefits once converted and is a day shift opportunity at a top employer in the country.

Pay and Benefits

The pay range for this position is $45.00 - $55.00
