POSITION PORPOSE

  • The ED Case Manager coordinates actions within the emergency center admission process that provide for patient's receiving the right care at the right time, in the right place.

  • This position is responsible for reviewing the clinical appropriateness of admission and for the communication of admission details to the Patient Access team.

  • Part-Time non-benefited position scheduled 12 hours per week (or 24 hours per bi-weekly pay period). .3 FTE

WHAT YOU WILL DO

  • Facilitates actions necessary for admitted or emergency center patients to receive the right care in the right place at the right time.

  • Assess all patients that qualify as a readmission. Determine reason for return and implement alternative to admission plan when appropriate. Document readmission information/interventions in the Epic CM system.

  • Identifies barriers to discharging patients from the E.D instead of admission as observation. Develops strategies and initiatives to address identified barriers. Work with other disciplines to assist in resolution of barriers to discharge. Report findings to Acute Care Flow Leadership team.

  • Proactively consults with the EC physician to determine eligibility for admission. Facilitates appropriate documentation for inpatient admission as required by hospital and insurance criteria.

  • Proactively consults with the EC physician to determine appropriate admission status (inpatient or observation) and level of care required. Facilitates appropriate documentation of reason for admission including the treatment plan as required by hospital and insurance criteria.

  • Communicates medical necessity documentation requirements with attending physician as needed.

MINIUMUM QUALIFICATIONS

  • Current Michigan Nursing License

  • Graduation from accredited nursing program: Effective January 1, 2013 all new hires must have a BSN degree or must agree in writing to obtain their BSN within five (5) years from date of hire.

  • Experience: Three (3) years recent experience in case management, utilization review or a related field.

  • Certification: Case Management is desirable: Individuals serving as case managers for the health system shall obtain CMSA certification within three years from their date of employment in the role or be in the process of obtaining certification as a case manager.
