The role you’ll contribute:
Under the direction of the Health Management leadership team the Whole Health Coordinator is a non-clinical role responsible for educating their assigned patients regarding available health care benefits and community resources; and making referrals to AdventHealth and community resources as needed. The Whole Health Coordinator will have frequent contact with patients, provider offices, and members of the interdisciplinary, collaborative team. In addition, The Whole Health Coordinator actively demonstrates outstanding customer service, escalates issues, barriers/challenges promptly, and accepts responsibility in maintaining relationships that are equally respectful to all.

Performs skills necessary to create a high-quality patient experience, as reflected through acceptable satisfaction scores, quality monitoring, and patient feedback
Makes and receives phone calls; sends and receives emails; responds to emails and requests in a timely manner
Researches and distributes department approved community resources
Documents activities and progress in the electronic medical record in a timely manner and per defined policy and procedures
Keeps manager and other members of the collaborative care team informed of issues impacting access to care or barriers to achieving expected goals or outcomes
The expertise and experiences you’ll need to succeed :
Minimum qualifications:
High School Diploma or GED
At least two years of work experience in a healthcare or managed care environment
Preferred qualifications:
Bachelor’s in social work or
Community Health Worker or
Medical Assistant or
Licensed Practical Nurse
Certified Community Health Worker or
Medical Assistant or
Licensed Practical Nurse
