DEPARTMENT: Department of Mental HealthSALARY: $61,020LOCATION & HOURS: 175 Green St, Albany NY with Expansive Community Presence 8:00AM-4:30PMBENEFITS:► Paid Time-Off ► Tuition Reimbursement ► NYS Retirement► Health, Dental, Vision and Hearing ► Employee Assistance ProgramDESCRIPTION:Albany County Department of Mental Health is seeking applications to fill a vacancy for a Health Home Care Manager(Veterans) to serve as a mental health and wellness case manager for veterans and their families, focusing on providingcomprehensive support and assistance in accessing various mental health services and resources. The care manager willhelp veterans navigate the Department of Veterans Affairs offerings, including mental health counseling, wellness programs, and related community resources. This position will emphasize collaboration with the Dwyer Program, the March On Program, and other local initiatives to support the well-being of Albany County military service members and their families. This role aims to reduce avoidable challenges, such as homelessness and limited access to mental health support.Does related work as required. MINIMUM QUALIFICATIONS: EITHER:A. Graduation from an accredited college or university with a Master’s degree in Human Services or a closely relatedfield and two (2) years of mental health support experience providing direct services to veterans and/or their families;OR,B. Graduation from a regionally accredited or New York State registered college or university with a Bachelor's degreeand five (5) years of mental health support experience providing direct services to veterans and/or their families.NOTE:Preference will be afforded to individuals with Lived Experience.Please submit a resume and letter of interest with this application.
