CHC Housing Case Manager in Akron, Ohio

CHC Addiction Services is a non-profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction.

 

We are currently looking for a Housing Case Manager for our OMCDC program. The Housing Case Manager will provide support to housing residents with emphasis on teaching residents how to remain in permanent housing and will also provide billable hours to CHC Addiction Services patients.

 

Duties Include, but not limited to:

  • Facilitate/assist with the move in for residents
  • Meet with residents on a weekly/bi-weekly basis. (Meet in resident unit once per month)
  • Maintain Case Management files with the required forms and documentation.
  • Develop and implement a "life skills" curriculum.
  • Enforce the housing leases and policies.

     

Minimum Requirements:

  • A Bachelor's degree or two years related experience. 
  • Ohio license or credential that conforms to OhioMHAS regulations pertaining to counseling services preferred.
  • Basic computer and research skills. 
  • Familiar with community resources. 
  • Must maintain an insurable driving record and have a valid Ohio driver license.
